Small Abdominal Aortic Aneurysms: Should We Wait?
نویسندگان
چکیده
The proper management of small abdominal aortic aneurysms (AAAs), namely those under the threshold of 5.5 cm in diameter, has been under investigation for years. Risk of rupture for this group of AAAs is higher than the rest of the population, and specific factors have been associated with increased growth rate of small AAAs. This review aims to collect and present all available research data on the development and progress of small AAAs. Furthermore, the results of major randomized trials on proper treatment of such patients are discussed, and conclusions regarding interventional and conservative management are made. Drug Therapy for Small Abdominal Aortic Aneurysm
Dear Editor,Abdominal aortic aneurysm is often asymptomatic, less recognized, and causes considerable mortality and morbidity, if missed. The incidence varies from country to country and the occurrence is influenced by modifiable (smoking, coronary heart disease, hypertension, dyslipidemia, and prolonged steroid therapy) and non-modifiable risk factors (increasing age, male gender, and positive...
متن کاملAn Aortoenteric Fistula Arising after Endovascular Management of a Mycotic Abdominal Aortic Aneurysm Complicated with a Psoas Abscess
Mycotic aortic aneurysms account for 1–3% of all aortic aneurysms. The management of this disease is controversial. Since open surgical repair is associated with high morbidity and mortality rates, endovascular aneurysm repair is an alternative treatment method with promising early and midterm outcomes, although its long-term durability is unknown.Secondary aortoenteric fistulas may occur iatro...
متن کاملScreening for Popliteal Aneurysms Should not be a Routine Part of a Community-Based Aneurysm Screening Program
INTRODUCTION Several studies have found an increased incidence of peripheral aneurysms in patients with an abdominal aortic aneurysm (AAA). The aim of this study was to determine whether screening for popliteal aneurysms should be part of an AAA screening programme. SETTING A community-based AAA screening programme. METHODS The diameters of the internal abdominal aorta and both popliteal ar...
متن کاملDetection, management, and prospects for the medical treatment of small abdominal aortic aneurysms.
Small abdominal aortic aneurysms, up to 5.5 cm in diameter, are very common. Ultrasonography is the most cost-effective method of detecting these aneurysms and keeping them under surveillance, because the natural history is 1 of continued expansion. The expansion rate is in the range 0.25 to 0.35 cm/y and is fastest in current smokers. From a study of expansion rates, it has been possible to fo...
متن کاملMortality results for randomised controlled trial of early elective surgery or ultrasonographic surveillance for small abdominal aortic aneurysms. The UK Small Aneurysm Trial Participants.
BACKGROUND Early elective surgery may prevent rupture of abdominal aortic aneurysms, but mortality is 5-6%. The risk of rupture seems to be low for aneurysms smaller than 5 cm. We investigated whether prophylactic open surgery decreased long-term mortality risks for small aneurysms. METHODS We randomly assigned 1090 patients aged 60-76 years, with symptomless abdominal aortic aneurysms 4.0-5....
